• Reference form (obtain two references and either include them in your application packet or have them sent directly to Dr. Garrison, Dept. of Anthropology); references from professors or employers are preferred; references from friends, family, or neighbors are not acceptable)

• Accommodations for Students with Disabilities form
• Agreement for Participation in the Switzerland Program form
• Release, Waiver of Liability and Covenant Not to Sue form
• Tuition & Program Costs and Registration Information sheet
• Hope Information sheet
• Interpretation & Clarification of UGA Alcohol Policy as it applied to Study Abroad Programs information sheet
• Switzerland Payments and Refunds Policy information sheet
• Franklin Arts and Sciences Study Abroad Scholarship Fund (application deadline: Feb 2)

Non-UGA students must also apply as a Summer Transient Student: application forms are available from the UGA Admissions website.
